Manufacturer-direct pricing has collapsed to the point where FDA-approved medication competes directly with compounded pricing.
Foundayo (orforglipron), the oral GLP-1 the FDA approved on April 1, 2026, starts at $149 a month self-pay. Wegovy tablets start at $149 a month through NovoCare Pharmacy. Zepbound starts at $299 a month through LillyDirect against a list price of $499 to $1,086 per fill.
That changes the calculation completely. If you were choosing compounded medication purely on price, the price gap that justified the regulatory risk has largely closed. These are the best routes to the real thing.
FDA-approved medication has verified potency and purity, is eligible for insurance and manufacturer savings programs, qualifies for the Medicare GLP-1 Bridge program from July 1, 2026, and is not exposed to the regulatory uncertainty that now surrounds compounded GLP-1s. Eli Lilly selling its own medication directly. Foundayo starts at $149 a month, the lowest published price for any branded GLP-1 for weight loss, and Zepbound runs $299 to $449 self-pay against a $499 to $1,086 list price. There is no membership fee and no subscription — you buy per fill. If you do not have a prescriber, LillyDirect connects to independent telehealth at $29 a visit, and Walmart Pharmacy offers retail pickup from $349 a month.
Watch out for: Prices are dose-tiered, so the $149 and $299 entry figures rise as you titrate. Lilly products only — no semaglutide here.
Best when you have insurance
The only platform where the clinician, the pharmacy and the delivery network are all one company. A message consultation is $29 and a video visit $49, with no subscription required. Medication runs as low as $25 a month with insurance, or $149 to $299 cash-pay for Foundayo and Wegovy tablets. U.S. News rated it the highest-scoring GLP-1 telehealth company at 4.8.
Watch out for: No tirzepatide options are published, so no Zepbound or Mounjaro pathway here.
Best for semaglutide
Novo Nordisk's direct-to-patient channel, the semaglutide counterpart to LillyDirect. Wegovy tablets at $149 a month for the 1.5 mg and 4 mg strengths, $299 for 9 mg and 25 mg, and the injection from $399. Free home delivery on every dose, no membership fee of any kind, and commercially insured patients can get to as little as $25 a month through Novo's savings offer.
Watch out for: You need a prescription already — NovoCare does not employ prescribers. The 4 mg promotional price ends August 31, 2026 and rises to $199.
Best bundled care plus medication
A flat monthly fee for unlimited online care, plus GoodRx-negotiated cash prices on brand-name medication. Wegovy tablets at $149 a month for 1.5 mg and 4 mg, Foundayo from $149 matching the manufacturer price, and Zepbound vials at $299 to $449. Offers pharmacy pickup as well as delivery, which most telehealth platforms do not.
Watch out for: The $39 rate is introductory — GoodRx published a standard price of $119 a month. Budget on the standard rate if you plan to stay.
Best with no subscription at all
A $49 video visit with no monthly subscription whatsoever, and same-day pickup at thousands of retail pharmacies. Carries the full brand-name lineup — Wegovy, Zepbound, Foundayo and Saxenda — with medication from $149 a month. If you are stable on a dose, paying nothing between visits is a real annual saving, and in-store pickup removes cold-chain shipping risk entirely.
Watch out for: Every follow-up is another $49, which adds up during titration. Restricted to ages 18 to 64 and does not currently support Medicare patients.
Best for using your existing insurance
A virtual primary care practice that bills insurance and writes prescriptions to any pharmacy you choose. The $19.99 monthly membership is among the lowest anywhere, and live video visits with board-certified physicians beat form-only intake. If your employer plan covers Wegovy or Zepbound, this route can put the medication on your formulary at a copay.
Watch out for: The membership does not include medication. Without coverage you pay both the membership and the full cash price, making this one of the more expensive routes.
